Connected Cardiac Care
Program Summary
Connected Cardiac Care is a program offered to heart failure patients at risk for frequent hospitalizations. The program aims to reduce re-hospitalizations by improving patients understanding of their condition, and providing on-going nursing support and review of key physiologic parameters while the patient is at home.
Program Overview
Connected Cardiac Care was launched as a result of a pilot at Massachusetts General Hospital, which admitted heart failure patients after discharge from the hospital. Data from this pilot suggests that telemonitored patients have a lower re-hospitalization rate than those who are not.
Patients in the program monitor their heart rate, blood pressure, blood oximetry, weight and symptoms on a daily basis. Patients use telemonitoring devices to record and transmit their vital signs. Each patient's physician establishes parameters for which alert levels are set. A telemonitoring nurse reviews the information daily, and intervenes when necessary.
Connected Cardiac Care combines patient self-monitoring of physiologic data and symptoms, with nurse intervention to educate patients, help them understand the links between their daily life and their disease, and to coordinate care with their physician when necessary. The ability of the nurse to intervene or educate at the time of need is possible through the use of remote monitoring technology.
Connected Cardiac Care is currently offered to patients affiliated with Partners HealthCare. The program is offered by the Center for Connected Health and Partners Home Care.
